ABSTRACT

Grains account for a large proportion of the diet of rural residents in Tibet. The lack of selenium (Se) and zinc (Zn) threatens the population's nutrition and health. However, the intakes of selenium and zinc in grains remains unclear. To clarify the nutritional status of selenium and zinc consumed from staple grains of residents along the Yarlung Zangbo River in Tibet, 341 grain samples and 242 urine samples were collected, and 244 food frequency questionnaires were completed along the Yarlung Zangbo River in 2020-2021. The results showed that the selenium concentrations of 88.5% of self-produced tsampa and 80.8% of self-produced flour were lower than the grain selenium threshold (<25 µg·kg-1). The intake of selenium and zinc from staple grains (tsampa, flour, and rice) contributed 15.0% and 43.5% to the recommended nutrient intake (RNI) on average, respectively. A geographical detector model analyzed factors affecting urinary selenium and zinc levels. Selenium and zinc intakes in rice and flour, and dietary diversity score (DDS) were the main factors affecting urinary selenium and zinc (p < 0.01). Their interaction effects on urinary selenium and zinc were greater than those of a single factor. The staple grains of rural residents along the Yarlung Zangbo River were in a state of selenium deficiency. The zinc content of the staple grain purchased was lower than that of the main grain produced by rural residents. Changing the grain consumption pattern and adjusting the proportion of exogenous grains can improve selenium and zinc nutrition in residents.

ABSTRACT

Nowadays, fluorosis is the most prevalent endemic disease in Tibet and one of the most concerned public health problems in China; urinary fluoride is a common diagnostic indicator of endemic fluorosis. However, the spatial distribution and influencing factors of urinary fluoride content in Tibet remain unclear. This study aims to bridge this gap by geographically weighted regression (GWR), analyses of variance (ANOVAs), Geodetector, and stepwise multiple linear regression (MLR). To achieve this goal, this study first investigated fluoride levels in the fasting urine among 637 Tibetan residents from 73 counties in Tibet, the urinary fluoride level was chosen as an indicator of fluorosis, which can reflect the health impairment. Then we identified the potential factors that could influence the spatial distribution and individual variation of urinary fluoride levels from physical environmental and socioeconomic perspective, respectively. The results showed that urinary fluoride levels in Tibet were slightly exceeding the average value for adult urinary fluoride in China, and that residents with high urinary fluoride were mainly found in the west and east; those with low urinary fluoride were mainly found in the central-southern regions. And the urinary fluoride levels had a significantly positive correlation with water fluoride and a significantly negative correlation with average annual temperature. Urinary fluoride levels increased until age 60 and followed an inverted "U"-shaped trajectory according to annual household income (80,000 renminbi (RMB) was the inflection point); pastoralists had greater exposure to fluoride than farmers. Furthermore, the Geodetector and MLR showed that the urinary fluoride level was influenced by both physical environmental and socioeconomic factors. The socioeconomic factors such as age, annual household income and occupation had a greater impact than the physical environment on urinary fluoride concentration. These findings can provide scientific basis for preventing and controlling endemic fluorosis in the Tibetan Plateau and adjacent areas.

ABSTRACT

The Tibetan Plateau (TP) is known as the water tower of Asia, and the water quality has long been a focus of public concern, especially in the Yarlung Tsangpo River Basin (YTRB), a unique area that is climate-sensitive, geologically complex, eco-fragile, and densely populated. Thallium (Tl) is a typical metal that is more toxic than Pb, Cd, and As and often occurs in sulfide minerals. Although large-scale polymetallic sulfide mineralization developed in the YTRB, the geochemical dispersion and potential risk of Tl in aquatic environments of the YTRB remain poorly understood. In this study, the concentration, distribution, source, and health risk of Tl and associated metal(loid)s in the hot springs and surface water in the YTRB were systematically analyzed. The results showed that the trace elements (Cd, Cr, Zn, Cu, Al, Sr, Ni, Co, Mn, Pb) in water environments are within the recommended limits, except for Tl and As. Principal component analysis (PCA) and correlation analysis (CA) showed that the elements of Tl and As were positively related to each other in either both hot spring water and surface water, indicating their common origin. Spatial variations suggested that high levels of Tl and As observed in the north YTRB, which may be relevant to the reduction-dissolution of Tl (As)-bearing minerals and the magmatic hydrothermal system formed in the shallow part of the northern YTRB. Furthermore, source apportionment identified natural sources of Cu, Ni, Cr, Co, Mn, Zn, and Cd and anthropogenic inputs of Al and Pb. Exposure assessment studies have found that ingestion is the primary route of As and Tl exposure to local population, and balneological and bathing purposes do not constitute a human health concern. This study offers valuable insights into the risk of naturally occurring Tl enrichment being hidden in As-rich hydrosphere in the YTRB and other regions with similar geoenvironmental contexts.

ABSTRACT

This study investigated the bioaccumulation and transfer of heavy metals including Cd, Cr, Cu, Mn, Ni, Pb and Zn in soil-crop system in Lhasa, and assessed the health risks of the edible part of the crops. The results showed that the average values of Cd, Cr, Cu, Mn, Ni, Pb and Zn were 0.15, 44.55, 24.68, 532.40, 22.47, 38.18 and 73.99 mg kg-1 in natural soil, and 0.16, 46.93, 38.45, 559.13, 23.23, 40.03 and 83.29 mg kg-1 in cultivated soil, respectively. Highland barley and wheat had the strongest ability to accumulate Zn in grain, the BCF values were 0.24 and 0.27, respectively, significant differences in the distribution of metal contents in crop root, stem, leaf and grain were observed. Root presented larger accumulation capacity in most metals, Zn and Cu was easily transferred in the plant organs, most metals in this study presented difficult to migrate from root to grain. The transfer peak of most metals in soil-crop system appeared from stem to leaf. The concentrations of Cr and Mn in crop grains could be predicted according to the multiple linear regression models. THQ and HI values of heavy metals in edible parts of both highland barley and wheat were below the safety threshold of 1, indicating no detrimental effects posed to adults health. This study helps to understand the accumulation and transfer of heavy metals in soil-crop system in plateau region.

ABSTRACT

Dietary imbalances are an important cause of morbidity and mortality, both in China and globally. Abnormal element content in the natural environment and the unbalanced dietary structure of populations coexist in the Tibetan Plateau. This study analyzed the dietary and food consumption patterns of 617 Tibetan residents and their associated factors. Cluster analysis revealed three modes of dietary pattern; the food consumption scores (FCSs) of subjects in modes with relatively high consumption frequency of staple food and relatively singular dietary structure were the lowest. Although the FCSs of most subjects were acceptable (FCS > 35), subjects with relatively low FCSs were more dependent on locally cultivated highland barley that is probably low in selenium. Hierarchical linear models revealed both individual−family and regional factors were significantly related (p values < 0.05) with the food consumption of subjects as follows: age, travel time from township to county, and cultivation area of highland barley were negatively related; numbers of individuals aged 40−60 years and pork, beef, and mutton production were positively related. Individuals with secondary or higher education had higher FCSs. A single indicator may be incomprehensive in dietary and food consumption studies. For people with a relatively unbalanced diet, an analysis of the main foods they consume is critical. Dietary and food consumption patterns might have relatively large inter-regional and intra-regional variations; therefore, factors that influence it might be multi-level and multi-scale.

Drinking water is considered to be an important exposure pathway for humans to ingest trace elements; human urine samples are widely accepted as biometric substrates that can reflect human exposure to trace elements. The current study aimed at investigating the concentrations of trace elements including selenium (Se), arsenic (As), cadmium (Cd), chromium (Cr), copper (Cu), manganese (Mn), nickel (Ni), lead (Pb), and zinc (Zn) in drinking water and human urine in plateau region of China, determining the association among trace elements in drinking water and urine, and analyzing their associations with age and gender. The results showed that the majority of trace element concentrations were in the range of the World Health Organization (WHO 2011) guideline values, in both urine samples of male and female, and the median values were descending in the order: Zn > Cu > As > Se > Cr > Ni > Mn > Pb > Cd > Co. Selenium contributed to the excretion of As, Cr, Cu, Cd, and Zn in human body, group of 31-40 years appeared to present the greatest excretion ability in most of the trace elements. Weak positive correlations were observed between age and Mn in female urine samples, and negative correlations were observed between age and Se, As, Co, and Cu in male urine samples and between age and Co in female urine samples, respectively. Significant positive correlation was observed in As between drinking water and the whole human urine. In the same family, female seemed to show higher proportions of urinary As levels than male. This study will provide elementary information regarding trace element levels in drinking water and human urine in residents in plateau region of China and is helpful to provide reference for dietary nutrient trace element intake and effective control for local resident.

INTRODUCTION: To estimate the prevalence of Kashin-Beck disease (KBD) among children in 2017 in Changdu of Tibet. METHODS: We adopted a four-step recruitment to include children aged 7-12 years from seven identified historically endemic counties in Changdu. Posterior-anterior radiographs of right hand and wrist were taken and were graded at four sites (metaphysis, epiphysis, and bony end of phalanges and metacarpal and carpal bones). Two trained researchers independently read the films. Inter-rater reliability was assessed using weighted Kappa and percentage agreement. We fitted logistic regression model to examine the association of age, sex, and altitude of residential village with prevalence of KBD. We examined association between site involvement and severity of KBD using chi-square test. RESULTS: We recruited 13,573 children (mean age = 9.3 years, 48.40% girls) with a response rate of 95.81%. The overall prevalence of radiographic KBD was 0.26%. Luolong County had the highest prevalence (0.69%), followed by Bianba (0.26%), Basu (0.24%), Mangkang (0.14%), Zuogong (0.14%), Dingqing (0.07%), and Chaya (0.00%). A higher risk of radiographic KBD was associated with older age (P for trend <0.001) and girls (OR=1.86, 95% CI: 0.94, 3.70), but not the altitude of residential village (P for trend=0.957). Metaphysis was involved in all cases of KBD while lesions in epiphysis and bony end of phalanges and metacarpals were only observed in severe cases. CONCLUSIONS: The prevalence of radiographic KBD among children aged 7-12 years was low in Changdu compared with previous census data, suggesting the effectiveness of preventative measures. Key Points • In this study, 13,573 Tibetan children were taken X-ray films of their hands and wrists. • The prevalence of radiographic KBD among children aged 7-12 years was low in Changdu of Tibet. • The preventative measures against KBD launched by Chinese government were effective in decreasing new onsets of KBD among Tibetan children.

ABSTRACT

Kashin-Beck Disease (KBD) is one of major endemic diseases in China. In this study, we estimated the health loss from KBD in Qamdo district of Tibet using the years lived with disability (YLD) metric and investigated the influence of environmental selenium (Se) on it by multiple regression model. The results showed that YLD rates produced a different ranking of health loss of KBD from that produced by prevalence rates between Basu and Luolong County, with higher health loss from KBD (43.61 YLD/1000) but lower prevalence (17.86%) in Basu County. YLD rates in two counites were both highest for the 45-64 years age group. Compared with the prevalence rate, the YLD rate had a closer relation to environmental Se and was significantly negatively correlated with Se in both soil and highland barley. The multiple linear regression further revealed that Se contents in cultivated soil and highland barley were main influencing factors for the health loss of KBD, which could explain 90.5% of the variation in YLD rates. The information obtained highlights the significance of the YLD metric in exploring the environmental etiology of KBD and provides important information on which to base decisions on future prevention and control of endemic diseases.

OBJECTIVE: To estimate the prevalence of rheumatoid arthritis (RA) in the Tibet Autonomous Region (China). METHODS: A population-based cross-sectional survey was conducted on 1458 residents of Luoma Town, Tibet Autonomous Region, who were aged ≥ 40 years old. We interviewed participants using questionnaires, and rheumatoid factor (RF), anti-citrullinated protein antibodies (ACPA), and C-reactive protein (CRP) were determined. The identification of RA in this study was on the basis of criteria issued by the 2010 American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) guideline. RESULTS: Herein, 782 participants completed all items of RA. The overall prevalence of RA was 4.86%, and the prevalence was higher in women than that in men (7.14% vs. 2.56%, p = 0.005). The age-standardized prevalence of RA was 6.30% (95% confidence interval (CI) 4.20-8.64%), which was 2.46% (95% CI 1.04%, 4.10%) and 9.59% (95% CI 5.93%, 13.77%) in men and women, respectively. CONCLUSION: The prevalence of RA is relatively higher in the Tibet than that in other areas of China.

ABSTRACT

The health effects of drinking water exposure to inorganic arsenic are well known but are less well defined for dietary exposure. The rising concerns of arsenic risks from diet motivated this study of arsenic concentrations in highland barley, vegetables, meat, and dairy products to evaluate arsenic exposure source and to assess health risks among rural residents of Ngari area, western Tibet. Total arsenic and arsenic speciation were measured by inductively coupled plasma mass spectrometry (ICP-MS) and high-performance liquid chromatography combined with ICP-MS (HPLC-ICP-MS) respectively. Average total arsenic concentrations of 0.18 ± 0.21 (n = 45, median: 0.07 mg·kg-1), 0.40 ± 0.57 (n = 17, median: 0.15 mg·kg-1), 0.21 ± 0.16 (n = 12, median: 0.17 mg·kg-1), and 0.18 ± 0.08 (n = 11, median: 0.22 mg·kg-1) were observed in highland barley, vegetables, meat, and dairy products, respectively. Inorganic arsenic was determined to be the main species of arsenic in highland barley, accounting for about 64.4 to 99.3% (average 83.3%) of total arsenic. Nearly half (44.4%) of the local residents had ingested >3.0 × 10-4 mg·kg-1·d-1 daily dose of arsenic from highland barley alone, above the maximum oral reference dose recommended by the United States Environmental Protection Agency (USEPA). The inorganic arsenic daily intake from highland barley was 3.6 × 10-4 mg·kg-1·d-1. Dietary exposure to inorganic arsenic alone increased the cancer risk probability to 5.4 in 10,000, assuming that the inorganic arsenic in highland barley has the same carcinogenic effects as that in water.

OBJECTIVE: Gout and hyperuricemia are common public health problem. There has been no epidemiological survey of gout and hyperuricemia in Tibet Autonomous Region, the southwest of China. Therefore, we estimated the prevalence of gout and hyperuricemia in Luoma Town, Naqu City, Tibet Autonomous Region of China. METHODS: A population-based cross-sectional survey was conducted among 1458 residents of Luoma Town, Tibet Autonomous Region, age ≥40 years. We used questionnaires in face-to-face interviews, anthropometric measurements and serum uric acid test. Hyperuricemia was defined as serum uric acid level ≥7 mg/dl in men and ≥6 mg/dl in women. The definition of gout in this study was on the basis of new 2015ACR/EULAR classification criteria. RESULTS: Nine hundred eighty-nine participants completed all items of gout and 818 participants attended to be taken blood samples for serum UA levels test. The overall crude prevalence of gout and hyperuricemia was 0.30% and 1.83% respectively. It was more prevalent in men than in women (2.86% vs 0.75%, P = .034) in hyperuricemia group. Tibetan had a lower age-standardized prevalence of gout 0.26% (95% confidence interval (CI): 0%-0.60%) and hyperuricemia 2.05% (95% confidence interval (CI): 0.99%-3.44%) compared with the China Health and Retirement Longitudinal Study results. CONCLUSION: This is the first large-scale population-based survey to demonstrate the prevalence of gout and hyperuricemia of the middle-aged and elderly population in Tibet Autonomous Region, China. The prevalence of gout and hyperuricemia is relatively lower than other places in China, and that might be influenced by ethnicity, genetic and environment factors. These findings will be useful for the future researches and health care strategies.

ABSTRACT

Kaschin-Beck disease (KBD) is an endemic osteoarthritis, and the etiology is closely related with levels of trace elements in the human body. Currently, it is clear that the selenium (Se) status of children in KBD areas is lower than that in non-KBD areas in the Tibetan Plateau, whereas role of other elements are yet unknown. This study aimed to assess some essential trace elements (Se, Mo, Mn, Zn, Fe, Cu, Co, and Sr) in children using scalp hair as a biomarker, and 157 samples from school children aged 8-14 years old were collected from both KBD and non-KBD areas in Shigatse, Tibet. Se and Mo were measured by inductive coupled plasma mass spectrometry, and the other elements were determined by inductive coupled plasma optical emission spectrometry. Compared with the non-KBD areas, Se, Mo, Mn, Fe, Zn, Co, and Sr levels of children in KBD areas were found to be significantly different (P < 0.05); while in linear discriminant analysis, only Se and Zn were found to contribute to the KBD prevalence in the study area. The hair Se level of children in KBD areas ranged from 0.115 to 0.299 mg/kg, while in non-KBD areas it ranged from 0.135 to 0.519 mg/kg. The Zn content of children's hair was between 83 and 207 mg/kg in KBD areas, while it was 37 and 219 mg/kg in non-KBD areas. Lower Se and higher Zn levels in children in KBD areas was found when compared with non-KBD groups. In addition, Mo levels were found to be different between KBD areas and non-KBD areas on the opposite side of the Yarlung Zangbo River, but no close relationship was shown because there was no difference compared with the non-KBD area on the same side of the river. Our observations suggest that Se deficiency is still an important factor for the occurrence and prevalence of KBD, while the relationship between Zn and KBD needs to be further explored in the Tibetan Plateau.

BACKGROUND: The prevalence of brick tea-type fluorosis is high in Tibet because of the habit of drinking brick tea in this region. Brick tea-type fluorosis has become an urgent public health problem in China. METHODS: A cross-sectional survey was conducted to investigate prevalence of brick tea-type fluorosis in all districts of Tibet using a stratified cluster sampling method. Dental fluorosis in children aged 8-12 years and clinical skeletal fluorosis in adults were diagnosed according to the national criteria. A total of 423 children and 1320 adults participated in the study. Samples of drinking water, brick tea, brick tea infusion (or buttered tea), and urine were collected and measured for fluoride concentrations by the fluoride ion selective electrode method. RESULTS: The fluoride level in all but one of the brick tea samples was above the national standard. The average daily fluoride intake from drinking brick tea in all seven districts in Tibet was much higher than the national standard. The prevalence of dental fluorosis was 33.57%, and the prevalence of clinical skeletal fluorosis was 46.06%. The average daily fluoride intake from drinking brick tea (r = 0.292, P < 0.05), urine fluoride concentrations in children (r = 0.134, P < 0.05), urine fluoride concentrations in adults (r = 0.162, P < 0.05), and altitude (r = 0.276, P < 0.05) were positively correlated with the prevalence of brick tea-type fluorosis. Herdsmen had the highest fluoride exposure and the most severe skeletal fluorosis. CONCLUSIONS: Brick tea-type fluorosis in Tibet is more serious than in other parts of China. The altitude and occupational factors are important risk factors for brick tea-type fluorosis.

ABSTRACT

Previous studies have shown that the selenium (Se) deficiency is an important factor for the etiology of Kashin-Beck disease (KBD). Although KBD is presently controlled in most regions of China, it is still active in the Tibetan Plateau. The present study aimed to assess the nutritional status of selenium in school children by using the Se level in hair as a biomarker in KBD endemic areas of Lhasa in Tibet, China. Hair samples of 155 school children aged 6-15 years were collected in both KBD areas and non-KBD areas of Lhasa in 2013. The Se level in the hair samples was determined by inductive coupled plasma mass spectrometry (ICP-MS). The average concentration of Se in children's hair was 0.232 µg/g in KBD areas of Lhasa, which was significantly higher than the data reported decades ago. A significant difference in hair Se was observed between the boys (0.255 µg/g) and the girls (0.222 µg/g) in the studied KBD areas (P < 0.01, Mann-Whitney U test), but hair Se did not vary by age or region. School children in KBD endemic areas in Lhasa likely have improved Se status as a result of high Se content staple food substitution with the enforcement of Free Education Policy and Nutrition Improvement Plan in Tibet. Nevertheless, there were still 20.3 % of students with low Se status (hair Se <0.20 µg/g), which showed that Se status of school children was also partly affected by low Se environment in KBD endemic areas of Lhasa.
